To successfully process the application form we request you to kindly fill all fields of the application form.
Please scan & keep the following documents ready before proceeding to fill the application form:
1. Passport size Photograph of Child (as they look today),
2. Father and Mother's Photograph separately with face clearly visible.
3. Birth Certificate of the child
4. Copy of the latest progress report -10th Review report card or 9th Final Report card
5. Aadhar Card of the child
